We're delighted you've chosen to book with us. Please review our policies below as booking an appointment indicates our agreement to these terms.
Intake Form

New patients must complete the New Patient Intake Form at least 24 hours prior to their first appointment.

The intake form link is sent:
by email approximately 60 hours before your appointment, and
by text approximately 50 hours before your appointment (if a mobile number is provided). -
Please notify us promptly of any changes to your health status before your session.
Cancellation Policy & Rescheduling Policy

A minimum of 48 hours’ notice is required to cancel or reschedule an appointment.

Cancellations made with less than 48 hours’ notice, or missed appointments (no-shows), will be charged the full service fee.
Within 48 hours due to illness or emergency:

Please notify us immediately.

A cancellation fee may apply if the appointment time cannot be filled.

Exceptions may be granted at the discretion of Yaima Massage & Movement, PLLC (YMM).
Appointments Attendance

Late arrivals may result in a shortened session to accommodate subsequent appointments.

Full payment is required regardless of session length.

These policies apply to all circumstances, including illness, weather, or unforeseen events.
Payment & Booking Security

A valid credit card is required to secure all appointments.

Co-payments, co-insurance, deductibles, and any charges not covered by insurance are the patient’s responsibility and are due at the time of service.

Insurance benefits are verified as a courtesy; verification does not guarantee payment.

Alternative payment methods are accepted at the time of service.
Communication & Client Comfort

We welcome open communication regarding your preferences, goals, and needs.

Please inform your therapist immediately if you experience any discomfort during your session.

Both positive and constructive feedback are always appreciated and help us provide the best possible care.
Insurance Patient Agreement
-
Insurance coverage is a contract between the patient and the insurance company. Yaima Massage & Movement (YMM) is not a party to this agreement.

Insurance verification is a courtesy and does not guarantee payment.
-
Patients are responsible for co-payments, co-insurance, deductibles, and any services not covered by insurance.

Patient-responsible amounts are due at the time of service.
-
If a claim is denied or partially paid, the remaining balance is the patient’s responsibility.

A valid prescription from a qualified provider (MD, DC, PT, or other authorized provider) is required for insurance billing and must be updated
